The look is more than just the amount of skin on the eyelids. It is the result of a set of interacting elements:
- the shape of the opening between the eyelids
- the height of the free edge of the eyelid in relation to the iris (colored part of the eye)
- the position of the eyelid crease. there is a connection between the skin and the muscle that lifts the eyelid (called the levator muscle), which draws the skin into the eye socket when the eye is opened, creating a crease. In Western eyes, this fold runs parallel to the eyelashes, about 2 to 4 mm above them.
- The volumes of the fat compartments. There are 3, one in the eyebrow, another in the orbit held in front by a sort of curtain, the septum, and the last is the fat in front of the inferior orbital bone rim.
- the position of the eyebrow, which is more or less high in relation to the bony arch, which is itself more or less high in relation to the eyeball
Over time, changes will occur whose chronology and topography are specific to each individual.
The aim of the surgery will be to correct all the elements that have deteriorated, not just the amount of skin or excess fat. There is therefore no single aesthetic operation for the eyes, but a multitude of procedures that must be adapted to each patient.
